Purpose
This competitive solicitation is issued in order to assist the Aging and Long-Term Support Administration, Home and Community Services Division of the Washington State Department of Social and Health Services (DSHS) in seeking qualified contractor(s) for a pilot project for Navigation and Support Services (N/S) will be conducted in joint efforts with AIRS standards for Information and Referral Services (I&R) services. The purpose of the Pilot project is to determine needs / gaps / barriers after the entry point of I&R services for those persons requiring additional N/S short term assistance to long-term supports. The Pilot project for N/S services model would be for those persons who fall outside of public, private, and local community resources or whom may need short term assistance in reaching those warm hand-offs and goals for support.
